I look after benefits at a Canadian university with about 6,000 academic and professional staff. A staff survey found 23% have regular caring responsibilities for an adult, usually a parent, and the unions have put paid carer's leave on the table for the next round.
We are thinking of five paid days a year, separate from sick leave and from the statutory family leaves, usable in half days. The open questions are whether five is meaningful and whether to ask for any proof of the caring relationship.

We offer ten days, self-declared, no evidence, at a hospital trust with 9,000 staff. Average use among those who take any is 3.6 days and only 8% of staff use it in a year, so total cost is far below what ten days sounds like.
We dropped the evidence requirement after the first year. People were being asked to produce a letter about a parent's dementia to get a half day off, and it soured the whole thing. Abuse has not been an issue we can detect.
